At the Juliet O Foundation, our vision is to support and uplift our cherished elderly community by providing essential care packages that help ease the challenges of hospital discharge.

Our uniquely curated packages contain essential items designed to facilitate a smooth and comfortable transition back into the community. These resources provide much-needed support until the individual regains greater physical independence, ideally with the assistance of loved ones, social care services, or care providers.

We also accept community referrals from social workers who identify individuals who would benefit from one of our essential care packages.

Why Our Work Matters

For many elderly individuals, hospitals provide a sense of comfort through regular social interaction with staff and fellow patients. This can make the prospect of returning home—often to an empty space—feel isolating and overwhelming, even when they are medically fit for discharge.

Research shows that 15% of elderly individuals discharged from hospital are readmitted—many of these cases could be prevented with the right post-discharge support. By addressing basic needs and promoting social reintegration, we aim to reduce unnecessary hospital readmissions and ensure that no elderly person transitions home without the essentials they need to feel safe, comfortable, and supported.

Our Founder

Juliet, Founder

With over 30 years of frontline experience in health and social care, Juliet Omoregie has dedicated her career to advocating for and supporting vulnerable individuals, particularly the elderly. Her extensive background spans various caregiving roles, where she has made a meaningful difference in the lives of those in need.

Beyond her work in the UK, Juliet has played a significant role in economic empowerment initiatives in Africa, creating job opportunities and supporting individuals in establishing their own businesses. She is also deeply committed to children’s education, offering financial assistance for school fees and helping to break the cycle of poverty.

Motivated by her passion for community support, Juliet founded the Juliet O Foundation to ensure elderly individuals receive the care and resources they need to transition smoothly back into their homes and regain their independence.
